To find a provider in the PPO network, visit azimuthrisk.com and click on the “Provider Search” link. Provide your insurance ID card during your visit to ensure direct billing.

Claim Incurred Internationally
Members may use any provider of their choice, as Azimuth does not operate through an international PPO network.
Pre-notification Requirements
Pre-notification is mandatory for certain procedures and services, such as hospital admissions, surgeries, MRIs, CT scans, and emergency evacuations. Failure to pre-notify may result in a 50% reduction of eligible expenses.

- Pre-notification does not guarantee approval of benefits.

Claim Submission & Billing
Most international claims are paid upfront by the member and submitted for reimbursement. Direct billing may be arranged upon request but depends on the provider’s discretion

Please note, direct billing agreements are at the discretion of each individual provider. Members must submit their claims for reimbursement by email or fax within 90-days of the date of service.
What You Need to Submit a Claim:
- Itemized bill with provider’s name and address.
- Patient name, diagnosis, and service dates.
- Breakdown of procedures and billed amounts.
- Receipt of payment
- Medical report (if requested).
- Completed Azimuth Claim Form (available in your fulfillment kit or at azimuthrisk.com).
Submit claims via email or fax within 90 days of service. Missing information may result in denial, and corrections must be resubmitted within 90 days.
Claim Approval & Reimbursement
Once approved, reimbursements are paid in USD via check or wire transfer. Wire transfers incur a fee:
- $35.00 international
- $25.00 domestic

Find a PPO Provider
Search for UHC PPO providers on www.azimuthrisk.com . Follow these steps:
- Select “Passport to Healthcare Primary PPO Network.”
- Click “Continue” and search by criteria or categories.
Confirmation
At appointments, confirm with the provider that they are listed in the UHC PPO network and provide your ID card. PPO providers should not request upfront payment. If needed, contact Azimuth for assistance.
